Comarch eligible patient is admitted to the hospital for a complication with their chronic illness. The administrator inputs the patient into the EMR, and flags them as eligible for the Comarch RPM
system.
Once the patient is flagged as eligible for the Comarch RPM system in the EMR, a social worker from the patient’s ACO receives an alert to visit the patient and add them to their Comarch dashboard.
The social worker visits the patient, educates them about the Comarch RPM system and schedules a home visit upon discharge to set up the RPM devices.
During the home visit, the social worker will install and teach the patient how to use the tablet and RPM devices. They will set up automatic reminders for the patient, program the devices with patient-specific clinical thresholds as directed by their provider, and answer any questions the patient has.
Each time the patient uses their RPM devices, the outcome is automatically added to their Comarch tablet and to their profile on their provider’s dashboard. If the outcome is outside of the clinical threshold set by the provider, the provider receives an immediate alert.
Since the doctor is alerted to changes in the patient’s condition, they are able to immediately intervene and prevent further complications. Using the Comarch RPM system, they can set up a telehealth call with the patient, an office visit, or message the home health team that the patient requires a home visit.
